Initial Consultation

$399 | 45 minutes

All new patients to the practice are required to complete a 45-minute initial consultation, which includes a comprehensive evaluation of their health history, current concerns, laboratory needs, and goals to develop an individualized plan of care.

***If you are a previous patient of Deb’s from another practice, you are not subject to the initial consultation requirement and may instead schedule the appropriate follow-up care appointment.***

**Any other specialized consultations outside the initial consultation and programs are based on the time needed to discuss the concern and provide recommendations. Fees are discussed prior to scheduling each appointment.

Integrative Primary Care & Annual Wellness

$420/year - This program fee is paid in full at enrollment and covers 12 months of care. The program does not automatically renew. Annual Wellness is included for patients currently enrolled in one of our six-month focused programs.

• Review and monitoring of laboratory results every 6 months

• 15–30-minute lab review appointment with personalized recommendations

• Ongoing oversight of thyroid, metabolic, and lipid health

• Routine preventive care

• Secure patient portal access for care-related questions and communication

*For established patients seeking ongoing health and wellness monitoring who are not participating in any of the six-month programs.

Six-Month Focused Programs

Hormone Optimization & Sexual Wellness

Six-Month Program
Payment option: $594 paid in full or 6 monthly installments of $99

Personalized hormone management for women and men, including ongoing monitoring and treatment adjustments.

• 30-minute follow-up appointments

• Laboratory monitoring and review approximately 6 weeks after initiating hormone therapy

• Laboratory monitoring and review every 6 months thereafter

• Medication management and treatment adjustments, when clinically appropriate

• Secure patient portal access for treatment-related questions and communication

• Annual Wellness included

Weight Management & Metabolic Health

Six-Month Program
Payment option: $594 paid in full or 6 monthly installments of $99

Medical management focused on weight and metabolic health. GLP-1 therapy, including individualized microdosing, is available if appropriate to the patient and prescribed by a licensed clinician.

• 15-minute follow-up appointments monthly until an effective medication dose is reached

• 30-minute follow-up appointments every 6 months thereafter

• Laboratory monitoring and review every 6 months

• Medication management and treatment adjustments, when clinically appropriate

• Secure patient portal access for treatment-related questions and communication

• Annual Wellness included

Healthy Aging & Performance Optimization

Six-Month Program
Payment option: $594 paid in full or 6 monthly installments of $99

Personalized medical management focused on optimizing health, vitality, performance, and healthy aging. Treatment plans are individualized based on health history, symptoms, laboratory findings, and personal goals. Peptide therapy may be considered as one treatment option when clinically appropriate.

• 30-minute follow-up appointments

• Laboratory monitoring and review approximately 6 weeks after initiating therapy, if clinically indicated

• Laboratory monitoring and review every 6 months thereafter

• Medication management and treatment adjustments, when clinically appropriate

• Secure patient portal access for treatment-related questions and communication

• Annual Wellness included
